FORMS SUMMARY (3+1)
There are a total of 3 required forms (see box at right) plus team dues. Players WILL NOT be allowed to participate in league games until all forms and dues are submitted.

ABOUT FORMS & DUES
Filling out & returning all required forms is a necessary and critical part of club participation. They serve to register each player with USA Rugby so that he is eligible for competition. Dues are required to cover costs associated with club operations. Thank you for doing this quickly and efficiently.

Waiver Form
The liability waiver form must be signed and returned ASAP.
